**Job Description:**
This is an exciting opportunity to join our cardiovascular service
line team and provide care to our pediatric heart patients and
their families.
This Nurse Navigator will work with two other Nurse Navigators and
a social worker and deliver increased communication with referring
providers, and most importantly, serve as a dedicated concierge
service to our patient and families who require subspecialty care.
This professional will have the opportunity to work throughout
Primary Children's Hospital, in all areas of cardiovascular care
and focus on kids pre-during, and post heart interventions as well
as coordination for subspecialty clinics.
We are a highly multidisciplinary team giving you the opportunity
to collaborate with physicians, nursing staff, allied health
professionals, and other APPs within the Heart Center. We are a
fast paced unit who takes pride in delivering excellent care as a
cohesive team.
This position will have a dual reporting relationship to the
Service Line Director and Division of Cardiology Administrative
Manager.
Patient / Family Facing:
+ Expedite and facilitate communication for patient consultation,
referrals, second opinions, and transfers.
+ Follow Cath / EP patients throughout care continuum, including
inpatient admissions, and collaborate with inpatient care
management resources.
+ Spearhead the process and organization for scheduling through
different components of Heart Center.
Clinician Facing:
+ Coordinate care with referring clinicians and referral patients
to simplify access, improve communication and enhance the patient/
family experience; Coordinate a plan of care for these heart center
patients
+ Provide referring physician with updates and pertinent medical
information.
+ Act as point person for referral base (patient/ families/
PCPs)
+ Review all outbound marketing (for providers and patients) and
patient educational materials
+ Expand business and referral relationships, working with heart
center leaders to support the growth of strategic services
+ Collaborate closely with Marketing and Communication and
Destination Services on generating strategic business and marketing
plans for the Heart Center
+ Broaden the reputation of the Heart Center through a variety of
educational events, summits, symposia for referring/future
referring provider
**Hours** : Full Time - 40 hours per week
**Minimum Qualifications**
+ Bachelor's Degree in Nursing (BSN) from an accredited institution
(degree will be verified).
+ Current Utah Registered Nurse License.
+ Basic Life Support (BLS)
+ Five years clinical (direct patient care) experience.
**Preferred Qualifications**
+ Master's degree in nursing or a related field.
+ Experience in quantitative and qualitative analysis and research
methods.
+ Prior leadership/supervisory experience.
+ Two years of clinical experience with cardiology.
**Physical Requirements:**
Ongoing need for employee to see and read information, labels,
monitors, identify equipment and supplies, and be able to assess
patient needs.

Frequent interactions with patient care providers, patients, and
visitors that require employee to verbally communicate as well as
hear and understand spoken information, alarms, needs, and issues
quickly and accurately, particularly during emergency
situations.

Manual dexterity of hands and fingers to manipulate complex and
delicate equipment with precision and accuracy. This includes
frequent computer use and typing for documenting patient care,
accessing needed information, etc.

The hourly range for this position is listed below. Actual hourly
rate dependent upon experience.
$40.67 - $58.98
