Job Description:This Advanced Practice Provider will join our
multidisciplinary geriatric medicine team at Cottonwood Clinic in
Murray, Utah. As part of the team, you will support and be
supported by a lead geriatrician, MHI, pharmacy, and nurse
Once fully trained and oriented, this APP will quickly build their
patient panel. Due to the high patient volumes in this clinic, to
be successful in this position, we highly prefer an APP with three
or more years experience in primary care management.
As a Geriatric Medicine APP, you will provide care to patients 75
years of age and older. Many of these patients will have multiple
chronic conditions, requiring frequent office visits to manage.
In this field you will develop long term relationships with your
patients as you assist them on their journey for a healthy life.
Establishing these long-term relationships is rewarding and you
often treat multiple family members and learn how to care for those
in your community. The emphasis is on keeping people well, not just
treating a disease state. You will focus on value-based care
initiatives and will be the primary resource for your patients
regarding their overall physical and mental health.
Some of the chronic conditions you will manage are: Diabetes,
dementia, hypertension, congestive heart failure, chronic kidney
disease, COPD/Asthma, Depression/Anxiety, Sleep apnea, GERD/IBS,
stroke/TIA, Anemia, along with preventive care.
Cottonwood Clinic is open Monday - Friday. This APP will typically
work four 10-hour days per week (M,W,Th,F) and will take
after-hours call on an as-needed basis.
This position is full time salaried and fully benefits-eligible,
which includes generous continuing medical education funds to
support you in your growth as an APP.Scope
Physician Assistant - Professional Accountability: Practice within
scope of licensure as defined in the Utah Physician Assistant
Act/Rules (58-70a, R156-70a), national certification, education,
training, and consistent with Intermountain Healthcare approved
privileges and/or other aspects of the credentialing process.
The professional activities and conduct of a PA in diagnosing,
treating, advising or prescribing for any human disease, ailment,
injury, infirmity, deformity, pain or other condition dependent
upon and under the supervision of a supervising physician.
Position Specific Duties:
This position is accountable for the provision of patient care and
other duties as permitted by licensure and defined within
Credentialing and Privileging documentation (hospital),
Credentialing and Scope of Practice (Medical Group), Consultation
and Referral Plan (NP), Delegation of Services Agreement (PA)
(DOPL) or other similar documentation as appropriate.
Performs patient assessment to obtain a complete and accurate
record of the patient's clinical information and medical symptoms.
Orders tests and exams to obtain additional information needed to
ensure proper and complete care of patient.
Based on assessment, diagnoses patient needs and determines a plan
of care. Interprets patient care tests and exams to obtain needed
information to guide care.
Provides for the treatment of the patient (and oversees the
treatment provided by others) including appropriate prescriptive
practice to achieve optimal clinical outcomes for the patient.
Utilizes advanced specialty knowledge to ensure that the care
provided is appropriate, effective, timely, and cost efficient.
Performs diagnostic, therapeutic and/or procedural care as defined
by Scope of Practice.
Appropriate and timely documentation of clinical care. Codes and
bills accurately per established protocol and procedure. Ensures
that records are detailed and complete and comply with related
standards and regulations.
Conducts and/or coordinates education to increase the knowledge of
patient, patient family, and others. Provides appropriate
instruction based on patient needs to support the continuum of
Trains other staff in areas of clinical expertise. Provides
teaching of students as appropriate to increase their clinical
knowledge and ensure safe patient care.
Stays abreast of current trends in healthcare practice and
participate in continuing medical education to ensure optimal
clinical knowledge and effective patient care skill.
Coordinates and collaborates patient care efforts with other care
Communicates clinical information timely, accurately, thoroughly,
and appropriately to other providers of patient care.
May collaborate with other care providers in the effective sharing
of practice expertise.
May participate in research and utilizes evidence-based practice to
promote quality patient care, protocol development, care process
model development, and best practice standards for assigned patient
Supports professional development through research processes.
Current state license as a Physician Assistant.

Eligible for state Controlled Substance License II through V.

Eligible for Federal DEA Certificate II through V.

Basic Life Support (BLS) Certification for healthcare
- and -
Other certification as required (e.g. ACLS, PALS, TNCC, NRP).
Master's degree or higher.
Applicable certification from a recognized certifying organization
such as the National Commission of Physician Assistants.
Applicable experience as defined by position leadership.
