Licensed Clinical Social Worker (LCSW) or (LISW) for Hill -Air Force BasePrimary Care Behavioral Mental Health setting

• Full-Time, Civilian Contractor position
• M-F, 730am-430pmThe LCSW will provide focused evidence-based assessment and intervention (primarily cognitive, behavioral, motivational, and acceptance-based approaches) in primary care under the Primary Care Behavioral Health (PCBH) model of service delivery for a range of problem presentations including but not limited to depression, anxiety, substance misuse, stress, insomnia, pain, obesity, diabetes, tobacco use and behaviors impacting health. - Requirements:
  • Master's Degree in Social Work, -fully accredited by the Council on Work Education (CSWE)
  • Minimum 1 (one) year -experience as an Independent practitioner of clinical social work in a mental health setting
  • The optimal candidate will have experience with a population health approach and motivational interviewing skills.Duties:LCSW will regularly engage in behaviors to increase the number and variety of referrals they receive from primary care managers (PCMs) to reach the utilization goal by maximizing opportunities to educate and train PCMH staff members on appropriate referrals and collaborating with the healthcare team to expand their ability to effectively deliver healthcare servicesServices will be delivered in a manner consistent with a population health approach.,(e.g., 15- to 30-minute appointments; brief episodes of care; availability of same-day services, educational classes, team-based approaches to health service delivery and assessing/addressing population needs).
