Job Description:


Prepares, processes, and issues vital records and documents for Salt Lake County.

Preference may be given to applicants bilingual in Spanish/English.


One (1) year of general office skills.

Must become a notary public within one (1) year of hire

Due to the nature of this position, the successful applicant must pass a required pre-employment background check and subsequent mandatory background checks in accordance with current County Human Resources policy requirements.

It is mandatory that all new hires receive the Tdap (Tetanus, Diphtheria & Pertussis) vaccine before beginning employment or provide a copy of their immunization record prior to starting employment. The immunization record must show the vaccine name and date received.

Upon hire, a two-step Tuberculosis skin test will be required. The first will need to be placed prior to starting employment.

A valid Driver's License to operate a vehicle may be required.


  • Registers accurate death certificates after performing reviews and making necessary corrections Reviews, tracks, and reports monthly registered death reports.
  • Verifies accuracy before issuing certified copies of vital records. Greets customers and visitors and disseminates general information and assistance to the public and staff. Answers telephones, responds to inquiries, routes calls, takes messages, and provides information as assigned.
  • Verifies proper identification and collects and processes correct fees when completing client applications. Prepares and verifies receipt of cause of death reports to required agencies. Records mail requests received each day for birth and death certificates; completes appropriate information as required by County.
  • Monitors, orders, and delivers supplies and necessary applications.
  • Maintains files and archival storage of applications. Performs cashiering duties and prepares daily cash reports.
  • Performs billing and accounts receivable functions to funeral homes and documentation submittal to internal finance group. Insures that all certified paper numbers are properly recorded within the designated software systems.
  • Assists clients in preparation, processing, and notarizing affidavits or declaration of paternity forms for mail or walk-in clients; receives and records adoptions and court orders from the State; coordinates the exchange of affidavit information between the Utah Bureau of Vital Records and the Department .
  • Provides general information/assistance regarding the departments available outreach services, applications, and education.
